1. Fill out application and gather all required documents at the Reg-4 Dropbox link below.
2. Schedule an appointment by e-mail to submit your completed application with form of payment. Only applications that are completed will be accepted.

3. The deadline to submit an application is 12:00 PM, the Thursday prior to an exam.
